Types of Reconstruction

The reconstruction process will vary depending on the size and location of the defect.  Common techniques used for reconstruction include the following:

Complex Closure

This method is used for smaller defects where there is enough loose skin nearby. The surrounding skin is moved and stitched together, typically leaving a straight-line scar.

Skin Graft

In this procedure, thin skin is taken from another part of the body and placed over the defect. A bolster is applied to hold it in place while blood vessels grow into the graft. It’s often used for nasal defects or large wounds where local skin isn’t available.

Adjacent Tissue Transfer of Local Skin Flap

This approach uses nearby skin that is moved to cover the defect while staying attached to its original blood supply. Techniques like rotation or advancement flaps help achieve a better match in texture and color.

Pre-Operative Consultation

Since the extent of the defect isn’t known until after Mohs surgery, most patients meet with us on the day of the procedure to discuss reconstruction. However, we’re happy to schedule a pre-op consultation in advance to review possible options if helpful.

Anesthesia

For small defects that will not require significant dissection, in-office repair under local anesthetic may be an option. However, for more extensive defects, defects on the nose, or for patients who prefer sedation, procedures will be performed under anesthesia in an operating room. This anesthesia may be a general anesthetic or sedation only, depending on the procedure.

Post-Operative Care and Recovery

Following surgery, patients will be allowed to go home the same day. Detailed post-operative care instructions will be provided. Discomfort will frequently subside over the next few days. Most patients are back to their normal daily activities within 1-2 weeks, but it is important to avoid strenuous activity for several weeks to ensure full healing.

Results and Scarring

Stitches are usually removed about a week after surgery, and the area often looks well-healed within a few weeks. Early on, scars may appear raised or discolored, but they typically soften and fade over the next several months. Proper scar care will be discussed during your visit, and avoiding sun exposure is key to helping scars heal well.
